Responsibilities
The Human Relations Office oversees and coordinates all employee related matters for the City of Lowell, including employment opportunities and benefits.

APRIL 2, 2025 to MAY 1, 2025
This is your annual opportunity to enroll, disenroll, make changes to plans, and add/remove dependents to your benefits. Plan is effective July 1, 2025.

Delta Dental Plan Information
DENTAL ENROLLMENT/CHANGE FORM:
*** Plan Update *** Effective July 1, 2025, Dependents are eligible for coverage up to the age 26. Proof of full-time student status is no longer required.
IMPORTANT NOTE REGARDING DEPENDENTS AGE 19 to 26: If a dependent lost coverage under your plan because they aged out or were no longer eligible as a full-time student, you have the option to re-enroll them. To do so, you’ll need to complete the necessary enrollment form.

Vision Insurance now offered through EyeMed
Coverage will be effective July 1, 2025.

EXISTING MEMBERS CURRENTLY ENROLLED IN FSA Instructions:
Re-enrollment is not automatic. It needs to be completed each plan year and must be done manually through the account portal. Visit Cafeteria Plan Advisors at https://cpaemployee.lh1ondemand.com/ and follow the login instructions to complete the process.
Can I use MyGICLink Member Benefits Portal for Annual Enrollment to change my plan?
- Yes, Existing GIC members can update GIC Medical benefits using MyGICLink Member Benefits Portal.

- Enrollment/change forms can also be submitted in person or via email: HRBenefits@lowellma.gov
- Dental and Vision enrollment and plan change forms should be submitted directly to the Human Relations Office. We do not participate in the dental and vision plans offered through the GIC. The City has our own plans offered by Delta Dental and EyeMed Vision

| If adding a spouse to medical/dental a copy of the marriage certificate is required |
| If adding a dependent to medical/dental a copy of the birth certificate is required |
| School Dept employees enrolling for the first time will need to obtain a Benefit Eligibility Form from the School Dept HR Office |
| All changes become effective July 1st |
| Deductions begin the first week of June |
| Be sure to check your deductions on your payroll checks for accuracy. Any discrepancies should be immediately reported to HR-City Hall |
